Dear Ambrose Community

We have now been advised by Alberta Health Services that Ambrose will be the first special H1N1 Clinic to happen here in Calgary.

Alberta Health Services (AHS) is offering special clinics to groups who want to facilitate the vaccination of their people.  AHS has launched this program on a cost recovery remodel and will be charging $15/person. These fees cover the cost of the nurses, logistics, overhead, and travel.

Because Ambrose feels strongly about protecting our community, all students and employees, spouses of students and employees, and children of students and employees 16 years of age or older, will be eligible to get the vaccination for $5 per person.

In order to participate in this clinic Ambrose Community members must sign up for appointments by going to our intranet SharePoint http://orange/Lists/H1N1%20Clinic/ and clicking on the schedule doc and entering their full name i.e., Jonathon Doe in the appropriate timeslots.

Individuals without access to SharePoint can phone 403-410-2000 at extension 0 or go to the Ambrose Reception desk to be signed in manually.

Please note the following important information:

  1. The clinic will be held in Airhart Theatre
  2. Individuals must sign up before 4pm, Wednesday November 25 to participate.
  3. $5 CASH ONLY will be collected at the clinic prior to inoculations.
Sorry but we cannot accommodate credit, debit, payroll deduction, or student account payment.
  4. Failure to show up for your appointment will result in the full $15 being charged to the "no show" individual.
  5. Ambrose ID must be provided by students and employees.
  6. Photo ID must be supplied by spouses and children 16 years and older.
  7. This is not a mandatory initiative. Please do your own research and make an independent decision whether or not to participate.
